Policies with Client Intake Form

Physical Therapy and Individual, One on One Class Policies:

1. Payment is prepaid via my website https://ritanaomi.com/book-a-session.
2. Individual Sessions are referred to as Individual Classes. Individual Classes are done in person or by phone. If in the event you are unable to show up physically to your appointment, and it is less than a 24 hour notice, a phone session will be conducted.
3. Fee: $350 for a physical therapy evaluation or for first time appointments. The individual class fee
is $300 unless we go over the regularly scheduled time. Sessions are in general 45-50 minutes in
length with more time spent for initial appointments.
4. Allow for time to get to your appointment as well as time for parking, time will not be made up at the end of the session in the event of lateness.
5. Cancellations: The full fee will be assessed for cancellations within the 24 hour period of the appointment.
6. For insurance reports, case reports, or activity requiring extra time outside our regularly scheduled time, the above rates apply and will be billed directly to you.
7. For a copy of your records: $30
8. $35 charge plus any fees incurred for payment that does not go through at time of session.
