Intake and Policies Form
Policies for Individual and Group Healing Classes:
- Payment
- To reserve your session, payment must be received.
- Payments are prepaid by zelle (contact Rita) or venmo @ritanaomimoran.
- Individual Sessions (Referred to as “Classes”)
- One-on-one healing sessions are referred to as Individual Classes and may take place in person or by phone.
- If you are unable to attend in person and provide less than 24 hours’ notice, the session will automatically be conducted over the phone.
- Fees & Sliding Scale
- The fee for an initial appointment (first-time session or physical therapy evaluation) is $300.
- Follow-up Individual Classes are $250 per session unless the session extends beyond the scheduled time.
- A sliding scale of $225–$300 is available for individuals experiencing financial hardship. Please contact Rita before you book an appointment.
- Sessions typically last 45-50 minutes, with additional time for initial appointments.
- Cancellation Policy
- Cancellations made within 24 hours of your appointment will be charged the full session fee.
- Arrival & Lateness
- Please allow extra time for parking and arrival. Time lost due to lateness cannot be made up at the end of the session.
- Medicare & Insurance
- Integrative Wellness, LLC (IW) and its providers (to include Rita Naomi) do not accept Medicare for physical therapy services. If Medicare is your primary insurance and you require physical therapy, IW will refer you to an appropriate provider who accepts Medicare or insurance.
- Some clients choose to work with IW as part of our Wellness and Healing Services. These services are designed to improve well-being, mobility, and overall function through a holistic and integrative approach. Sessions may include movement therapy, breathwork, lymphatic work, and other wellness modalities that support healing and optimal health.
- All physical therapy, wellness, and healing services are paid for out of pocket. A receipt will be provided for each session; however, no superbill or invoice with medical diagnosis codes or CPT codes will be issued for Medicare-eligible clients. For Medicare-eligible clients, a receipt will be provided for wellness and health maintenance services.
Frequently Asked Questions
Q: Can I submit my invoice to Medicare to obtain a denial that I can then submit to my secondary insurance for reimbursement?
A: No. IW is not a Medicare provider. Submitting an invoice for wellness and health maintenance services will not result in a Medicare denial that can be used for secondary insurance reimbursement.
Q: Why doesn’t Integrative Wellness participate with Medicare?
A: By maintaining a non-participating status, IW is able to provide personalized and comprehensive care. This allows for the use of diverse and effective wellness techniques tailored to individual client needs.
- Administrative Fees
- Failed Payment Fees: A $35 charge plus any additional bank fees for payments that do not go through at the time of the session.
- Donation-Based Group Classes
- I offer mindful movement classes on a donation basis 1-2 times per week. While donations are appreciated, they are not required.
